    Colgajo tendinocutáneo vascularizado de dorsalis pedis: caso clínico

    Los grandes accidentes que afectan a la mano suelen cursar con daños en más de un tejido. Son frecuentes las exposiciones óseas, fracturas, lesiones tendinosas y pérdidas de cobertura cutánea, lo que supone un importante problema para la cirugía reconstructiva. Las técnicas quirúrgicas para afrontar estas lesiones múltiples pueden reunirse en tres grupos: A) múltiples procedimientos escalonados, B) procedimientos únicos parcialmente vascularizados, C) procedimientos únicos totalmente vascularizados. Aquellas en las que en un primer tiempo se realiza cobertura cutánea y en un segundo tiempo se procede al injerto tendinoso, acarrean problemas relacionados con rigideces, dificultad en el cálculo de la longitud del injerto tendinoso debido a retracción muscular y adherencias tendinosas. Un colgajo muy utilizado para la reparación de este tipo de lesiones ha sido el del dorsalis pedis; muchos autores rechazan su uso como colgajo debido a varios inconvenientes: anatomía variable de la zona donante, disección tediosa y sobre todo, elevada morbilidad de la zona donante. Sin embargo, puede ser usado como colgajo tendinocutáneo ya que proporciona cuatro tendones vascularizados de adecuada longitud cubiertos por completo de epitenón y unidos de manera holgada a una piel que es muy similar a la del dorso de la mano. Además, el tendón vascularizado reduce el problema de adherencias que ocurre con los colgajos tendinosos simples. Presentamos un caso de traumatismo severo de la mano con afectación completa de los tendones extensores.

    Micro-CT evaluation of bone grow concept of an implant with microstructured backtaper crestally and sub-crestally placed. Preliminary study in New Zealand rabbits tibia at one month

    The primary purpose of this study was to determine the accuracy of micro-computed tomography (micro-CT) as a novel tool for the 3D analysis of bone density around dental implants in tibia rabbits. Six male New Zealand rabbits were used in our evaluation. One Copa SKY® (Bredent Medical GmbH & Co. K.G.) with a 3.5 mm diameter by 8.0 mm in length was placed within 12 tibia rabbits divided into two experimental groups: Group A (crestal placement) and Group B (sub-crestal placement). The animals were sacrificed at four weeks. Micro-CT evaluations showed a high amount of bone around all implants in the tibia rabbit bone. There was an increased formation of bone around the Copa SKY implants, mainly in the implants that were placed crestally. The most frequent density found in most implants was a medullary bone formation surrounding the implant; the density three (D3) was the most common type in all implants. The 3D model analysis revealed a mean bone volume (B.V.) of 31.24 ± 1.24% in crestal implants compared with the 43.12 ± 0.43% in sub-crestal implants. The mean actual contact implant to bone (B.I.C.) in the sub-crestal group was 51.76 ± 0.86%, compared to the 42.63 ± 0.75% in the crestal group. Compared to crestal implants, the Copa Sky implant placed sub-crestally allows for the formation of bone on top of the neck, thereby stimulating bone growth in tibia rabbits.

    Posterolateral elbow dislocation: an all-arthroscopic reconstruction of the lateral ulnar collateral ligament

    The elbow is one of the most commonly dislocated joints. Although simple dislocations of the elbow usually resolve with conservative management, certain patients can experience residual chronic instability. Posterolateral rotational instability accounts for approximately 80% of elbow chronic instability cases. We propose an all-arthroscopic reconstruction of the posterior, or ulnar, fascicle of the lateral ligament complex using an autograft or allograft, performed with a 5-mm-thick and 8-cm-long graft. The graft is first inserted distally into the supinator crest with an Arthrex 4.75-mm SwiveLock implant and, finally, at its origin in the epicondyle, also with a screw of the same characteristics. Arthroscopic techniques create fewer complications. This procedure allows the surgeon to address intra-articular elbow joint pathology with less chance of wound complications and the ability to use bone anchors if desired.

    Ensayo clínico aleatorizado sobre la utilidad de la impresión 3 D en las fracturas intraarticulares de radio distal

    Objetivos Evaluamos la utilidad de la tecnología de impresión 3 D para la planificación preoperatoria en el tratamiento de las fracturas intraarticulares de radio distal en relación a la mejora de la técnica quirúrgica, los resultados radiológicos y clínicos. Material y métodos Un total de 30 pacientes con fracturas 2 B y C de la clasificación de la AO fueron intervenidos por un solo cirujano con placa volar, divididos en dos grupos de forma aleatoria, 15 de ellos mediante planificación convencional (radiología simple y TC) y 15 añadiendo un modelo 3 D de la fractura y la simulación previa de la intervención. Se registró el tiempo de la simulación, el tiempo quirúrgico en minutos, el tiempo de radioscopia en minutos, la pérdida de material expresada en tornillos desechados en quirófano. A los 6 meses de seguimiento se realizó una evaluación funcional de la muñeca basado en el cuestionario PRWE y análisis radiológico completo por un observador independiente y cegado. Resultados No se observaron diferencias estadísticamente significativas en el cuestionario PRWE (p = 0,22), tampoco observamos diferencias en los valores radiológicos, salvo en relación al escalón articular (p = 0,028) lo cual representa una significación estadística, pero en ambos grupos la mediana fue de 0,0 (0,0-0,0). Tampoco vimos diferencias estadísticamente significativas en los tiempos quirúrgicos (p = 0,745), de radioescopia (p = 0,819) o en la pérdida de material de síntesis (p = 0,779). Conclusión La impresión 3 D no ha mejorado los parámetros estudiados en relación con los pacientes intervenidos de rutina.

    Treatment of proximal radioulnar synostosis using a posterior interosseous antegrade flow pedicled flap

    The aim of this study is to determine the different therapeutic options described for the treatment of radioulnar synostosis, and report our experience with posterior interosseous antegrade flow pedicled flap with technical amendments. Two patients, who were treated with the designed flap, and with more than one year of follow-up, were reviewed. The technical innovations, end result and complications are described. In the two cases described, there was no recurrence of synostosis, which is the most frequent complication described in this condition, and no postoperative complications were observed. In the literature, many filler materials, from artificial to biological, free or vascularized, have been used the radioulnar space after excision of synostosis. The technique that provides the best results is the interposition of muscle or vascularized adipofascial flaps. The interosseous posterior antegrade flow pedicled flap is reliable, with a low morbidity, and is an effective alternative for the treatment of proximal radioulnar synostosis.

    A multifactorial, criteria-based progressive algorithm for hamstring injury treatment

    Introduction: Given the prevalence of hamstring injuries in football, a rehabilitation program that effectively promotes muscle tissue repair and functional recovery is paramount to minimize re-injury risk and optimize player performance and availability. Purpose: To assess the concurrent effectiveness of administering an individualized and multifactorial criteria-based algorithm (RA) on hamstring injury rehabilitation in comparison to employing a general rehabilitation protocol (RP). Methods: Implementing a double-blind randomised controlled trial approach, two equal groups of 24 football players (48 total) completed either an RA group or a validated RP group five days following an acute hamstring injury. Results: Within 6 months after return to sport, 6 hamstring re-injuries occurred in RP versus 1 in RA [relative risk = 6 (90% confidence interval: 1-35); clinical inference: very likely beneficial effect]. The average duration of return to sport was possibly quicker (ES=0.34±0.42) in RP (23.2±11.7 days) than in RA (25.5±7.8 days) (-13.8%, 90%CI: -34.0 to 3.4%; clinical inference: possibly small effect). At the time to return to sport, RA players showed substantially better 10-m time, maximal sprinting speed as well as greater mechanical variables related to speed (i.e., maximum theoretical speed and maximal horizontal power) than the RP. Conclusions: Although return to sport was slower, male football players who underwent an individualized, multifactorial, criteria-based algorithm with a performance- and primary risk factor-oriented training program from the early stages of the process markedly decreased the risk of re-injury compared to a general protocol where long length strength training exercises were prioritized.

    Revision of total knee arthroplasty with the use of patient-specific instruments: an alternative surgical technique

    Introduction: Accuracy in the placement of components in revision total knee arthroplasty (R-TKA) surgery is sometimes challenging. The applicability of patient-specific instruments (PSI) in knee surgery has progressively expanded to types of surgery other than primary arthroplasty. Could this assistive technology be used to facilitate accurate R-TKA surgery? The aim of the current manuscript is to describe this new application of PSI for revision of TKA-to-TKA and to provide a step-by-step technical guideline for use. Areas covered: We will describe the application and a detailed description of PSI technology to TKA revision surgery, step-by-step, from CT images acquisition for preoperative planning and PSI blocks production to the surgery. Expert commentary: The system can facilitate the accomplishment of the bony cuts for optimal implant placement and that can be useful in minimally altering the femoral and the tibial joint line. It is obvious that technology alone will not replace surgical skill and that accuracy of the system will also depend on the quality of the CT images and the ability of the software to prevent metal artifacts. Despite that, our initial results are promising and prove that the concept of applying PSI technology to the R-TKA surgery is feasible.

    Neurotizaciones Sensitivas en Cirugía de la Mano

    La lesión de los nervios mediano y cubital puede ocasionar daños permanentes y tiene consecuencias devastadoras. Los accidentes con instrumentos cortantes en ambiente laboral o doméstico y las lesiones del plexo braquial, constituyen las causas más frecuentes. En los casos de lesión del nervio mediano, recuperar la sensibilidad de la “pinza” (borde cubital del pulgar y radial del índice), es esencial para la reeducación de la función motora e incluso, puede considerarse un requisito previo. Asimismo, la lesión del nervio cubital, conlleva a déficit de sensibilidad del borde medial de la mano y produce falta de soporte para la manipulación con la pinza. En ambos casos, la mano está expuesta a traumatismos, heridas, quemaduras, etc. En el momento de la lesión y siempre que sea posible, debe realizarse una reparación primaria del nervio. Las avulsiones, las lesiones proximales y los grandes defectos nerviosos, obligan a una reconstrucción secundaria cuando el paciente se ha recuperado de la lesión aguda. Este estudio objetiva una actualización de la literatura.

    Impact of surgical instrumentation on hospital length of stay and cost of total knee arthroplasty

    Background: We aimed to analyze the impact of two different types of surgical instrumentation (conventional manual instrumentation (CI) and patient-specific instrumentation (PSI)) on length of stay (LOS) and objectify differences in cost. We hypothesized that there are no differences in the LOS and cost due to the instrumentation system used. Research design and methods: LOS was registered using inpatient admission data provided by the Institutional Management Control Department. We recorded the costs associated with each procedure that could be influenced by the use of one system or another during the in-hospital stay. We conducted a prospectively single-center cohort study of 305 TKAs. Surgery was performed with conventional CI in 122 cases and with PSI in 183 cases. Results: The mean LOS for the CI group was 4.29 days (SD 1.65) and 4.22 days (SD 1.26), for the PSI group. No significant difference among both instrumentation systems was obtained. When comparing global costs, the mean cost was slightly higher (without a significant difference) for the PSI cases (€3110.24 vs. €2852.7 for the CI cases). Conclusions: LOS and overall cost, in hospitals with a low annual TKA surgery volume, are unrelated to conventional or patient-specific instrumentation.

    Fractura-luxación transemilunar

    Presentamos el caso de un varón deportista de profesión, que tras un traumatismo durante su práctica deportiva habitual sufre una fractura-luxación transestiloidesradial- transemilunar, lesión que es poco común y que escapa de los patrones típicos. Tras reducción cerrada de urgencia fue intervenido quirúrgicamente realizando osteosíntesis del semilunar y reparación del ligamento escafolunar, con recuperación satisfactoria. Las fracturas-luxaciones carpianas son lesiones severas que pueden asociarse a múltiples patrones de lesiones ligamentosas y óseas. Bain añade el arco translunar (fractura del semilunar) para usarlo como complemento al modelo de clasificación de inestabilidades perilunares de Johnson de arco mayor-arco menor. Este tipo de lesión translunar no sigue el esquema descrito por Mayfield, aunque sí es una combinación de este concepto con una fractura del semilunar. En el caso que presentamos se produjo una afectación de los 3 arcos: fractura de la estiloides radial (arco mayor), fractura del semilunar (arco translunar) y lesión de los ligamentos carpianos (arco menor); esto aún no se ha estudiado biomecánicamente.
