Análisis de transferencia por los equipos de enfermería de pacientes complejos hospitalizados en Medicina Interna

Introduction: Currently, nursing teams lack standardised tools to ensure patient handovers during shift changeovers and to guarantee continuity of care, particularly for highly complex patients. This issue prompts reflection on shift changeovers with regard to patient safety. Objectives: To analyse the perceptions of nursing team professionals regarding the handover of complex chronic patients (PCC) hospitalised in internal medicine units in the Region of Murcia. Method: Multicentre, descriptive, observational, prospective and cross-sectional study with a quantitative approach. An adapted questionnaire was administered to nursing teams in internal medicine units across the Region of Murcia. Sociodemographic, work-related and perception variables were analysed using descriptive and inferential statistics (p<0.05) with SPSS 23.0. Results: Of the 244 participants, 85% believed they performed patient handovers correctly; however, 84.83% felt that handovers could be improved for more complex patients. The majority of participants were women (88.11%), over 40 years of age (68.4%), and working 12-hour shifts (82.4%). Significant associations were found between years of experience and the appropriateness of the handover location; between being a nurse and forgetting information after a night shift; and between working 12-hour shifts or holding a supervisory position and the need to contact others due to missing information. Conclusions: The findings reveal a positive yet insufficient perception of the handover process. Standardisation of information transfer for complex patients, together with the establishment of multidisciplinary teams, is required to strengthen patient safety and ensure continuity of care

Usefulness of lymphocyte-to-monocyte, neutrophil-to-monocyte and neutrophil-to-lymphocyte ratios as prognostic markers in breast cancer patients treated with neoadjuvant chemotherapy

Background: Nowadays, neoadjuvant chemotherapy (nCT) in breast cancer is more and more standardized, not only in advanced tumours but also in those for which there is an attempt to achieve breast-conserving surgery. In literature, we can find evidences of the relationship between several types of tumours and systemic inflammatory response. Our objective is to analyse the prognostic value of blood parameters (lymphocytes, neutrophils, monocytes, lymphocyte-to-monocyte ratio (LMR), neutrophil-to-monocyte ratio (NMR) and neutrophil-to-lymphocyte ratio (NLR) in breast cancer (BC) patients treated with nCT. Methods: A retrospective cohort of 150 breast cancer patients treated with nCT and subsequently with surgery was analysed. Data about the patients, histology, response to chemotherapy and peripheral blood values of lymphocytes, monocytes and neutrophils was collected, and used to calculate the LMR, NMR and NLR. Univariate and multivariate analyses were performed for the variables to see the relationship of the ratios to disease-free survival (DFS) and overall survival (OS). Results: Patients with high LMR (≥5.46) and low NLR (<3.33) were associated with a lower percentage of relapse (P = 0.048 and P = 0.015, respectively) and, above all, NLR was associated with a better survival (P = 0.024), being those factors that predict a good progress. Conclusion: High LMR and low NLR can be considered as favourable prognostic factors in BC patients treated with nCT.

Esófago de barrett y progresión a adenocarcinoma: estudio estadístico de los factores pronósticos en la malignización. Distribución en grupos de riesgo

OBJETIVO: 1. Determinar, según el estudio estadístico, que factores y características de los pacientes con EB suponen un mayor riesgo de malignización. 2. Basándonos en estos factores, establecer grupos de riesgo de malignización, que nos permitan determinar la vigilancia y el tratamiento más eficaz. - Riesgo bajo - Riesgo intermedio - Riesgo alto METODOLOGÍA: A todos los pacientes se les realizó anualmente una entrevista clínica presencial y una endoscopia digestiva alta con toma múltiple de biopsias, o antes del año si reaparecieron los síntomas. En caso de existir displasia de bajo grado, la endoscopia y tomas de biopsias se repitieron a los 6 meses; hasta obtener dos endoscopias consecutivas con biopsias sin displasia, antes de pasar al protocolo anual. A los pacientes intervenidos quirúrgicamente se les realizó, al año de la cirugía y luego cada 5 años, una manometría y pHmetría esofágica. Solo a un subgrupo (n=30, 43 %) de pacientes con tratamiento médico, las pruebas funcionales esofágicas se repitieron sin interrumpir la medicación de IBPs, para estudiar la eficacia del tratamiento médico. El estudio del reflujo biliar con el Bilitec 2000, al ser una prueba más reciente, no se pudo realizar a todos los pacientes (n=72, 49 %).
