    Adenocarcinoma of the paraurethral glands: a case report

    Adenocarcinoma of the paraurethral glands represents a very rare neoplasm of the urinary tract. Due to the rarity of this disease, there is no standard therapeutic approach. We report a case of adenocarcinoma of the paraurethral glands in a 56-year-old woman, presenting with abnormal serous vaginal discharges. The radiologic examination revealed a 5-cm mass around the urethra, which underwent surgical resection. After surgical resection, the histology revealed a moderately differentiated adenocarcinoma, probably arising from the paraurethral glands. One month later, a pelvic recurrent mass was radiologically diagnosed; consequently, an anterior pelvic exenteration with lymph node dissection was performed. Histological examination revealed a moderately differentiated adenocarcinoma, with glandular and micropapillary architecture, with multiple lymph node metastases. The absence of modifications such as urethritis cystic glandularis on the urethral mucosa, as well as the lack of a lesion in situ, associated with the immunohistochemical expression of PAX8 and negativity for GATA3 and S100p, suggested that the adenocarcinoma originated from the paraurethral glands rather than from the urethral mucosa. Post-surgery CT scans revealed no evidence of metastatic disease. The patient received 6 courses of adjuvant chemotherapy with carboplatin and paclitaxel. One year after the pelvic exenteration, because of inguinal lymph node progression, an inguinal lymphadenectomy was performed. Four months later, a TC-PET revealed a multidistrectual lymph node and a lung micronodule disease progression. Invasive micropapillary carcinomas have been characterized as a rare distinctive variant of carcinomas in several anatomic sites and are distinguished by a marked tendency to lymphovascular invasion, justifying the association with high-stage disease and poor prognosis. In the present case, both the poor prognosis connected with micropapillary structure and the lymph node involvement, encouraged adjuvant cisplatinumbased chemotherapy.

    Efectividad de la hidroterapia en atención temprana

    Objetivo Valorar la eficacia de la hidroterapia en atención temprana. Estrategia de búsqueda Revisión sistemática en bases de datos PEDro, PubMed, Enfispo, Dialnet, IME (índice médico español), Science Direct e Isi Web Of Knowledge mediante términos Mesh y el operador lógico AND, así como una búsqueda bibliográfica manual. Selección de estudios y datos Fueron seleccionados 5 artículos que cumplían los criterios de inclusión y exclusión establecidos. Resultados Todos los estudios cuantificaron una mejora tras la intervención, ya fuese en variables fisiológicas, en el comportamiento o en la movilidad funcional; sin embargo, la calidad metodológica era escasa, siendo dos de ellos estudios de un solo caso, otros dos presentaban un tamaño muestral superior pero sin grupo control y un quinto estudio carecía de muestra aleatoria. Conclusiones Son necesarios estudios de mayor calidad metodológica que permitan evidenciar los efectos reales producidos por la hidroterapia en este ámbito.

    Framing sport managers’ profile: A systematic review of the literature between 2000 and 2019

    The present study is a systematic review focused in sport managers’ profile and the main purpose is to map the literature of sport managers’ competencies and tasks from January of 2000 to January of 2019. The search was made on EBSCO, B-on, Web Science and Scopus using the following keywords: ‘sport managers OR sport directors OR sport administrators’, ‘sport managers AND qualifications’, ‘sport managers AND responsibilities’, ‘sport managers AND traits’, ‘sport managers AND roles’, ‘sport managers AND competencies’ and ‘sport managers AND characteristics’ in peer reviewed articles and dissertations. After applying the inclusion criteria, 65 studies were selected. Sport managers are, predominantly, men in their thirties and forties, with a degree in physical education. A sport manager must have competencies such as leadership, integrity, specific knowledge, resource allocation, authority’s delegation, employees’ motivation and innovative thinking to accomplish the most important and more time consuming responsibilities like planning/organization, coordination/evaluation or information.

    Influence of circadian rhythms on sports performance

    Chronobiology is the scienti!c discipline of study of biological rhythms, a term that has gained ground in the sports world. Recently numerous studies have indicated that the time of day in which sports are practiced in"uences the achievement of good physical performance. The aim of this review was to study the relationship between circadian rhythms and physical performance, accord ing to the latest published data. In addition, the physiological processes involved in the physical response and the di#erences according to the type of sport and athletes’ characteristics were studied. A bibliographic search was carried out through !ve databases (Pubmed, Scopus, Researcher Gate, Google Scholar, UOC Library), focusing on articles published in the last ten years and written in English and Spanish. 36 papers met the inclusion criteria. Body temperature is a factor that shows a circadian pattern with a marked peak in the later afternoon, time of the day at which physical performance is at its highest, i.e. speed, agility, distance covered, jumping power. The perception of e#ort is also higher in the afternoon. Regarding the chronotype, evening types seem to be the most a#ected to do sports out of their optimal time-of-day. The tendency shows more morning types as age increases. Training sessions should be planned according to the optimal time of day for each athlete. It’s essential to take into account individual chronotype. The desyn chronization of circadian rhythms can cause a decrease in physical performance.

    Presentamos la historia de la investigación y el análisis de una inscripción funeraria múltiple infantil que conservamos por dos dibujos del siglo XVIII, que erróneamente fue asignada a la tierra valenciana. En Águilas se documentan varias necrópolis tardorromanas, pudiendo ser la del Molino su lugar de procedencia. Por la referencia al concepto fidelis aplicado a la infancia podría datarse entre los siglos IV y V d.C.

    Nuevas aportaciones en psicología del deporte. Una mirada crítica sobre la última década de nuestra disciplina

    Cuando se habla de nuevas aportaciones científicas en una disciplina, nos parece básico conocer lo que "innovadoramente" se ha planteado hasta el momento de afirmar que estamos ante una nueva aportación. En este trabajo, se plantean los ocho últimos años de investigación en psicología del deporte en nuestro país (se analizan los trabajos a congresos desde el primero que recoge información del total de trabajos presentados, Valencia 1995), y se recogen los artículos de la Revista de Psicología del Deporte desde ese mismo año para igualar las propuestas posibles que se han realizado en ambas fuentes: así, se trabaja con las investigaciones presentadas en los últimos cinco congresos nacionales (1995-2003) -el próximo se celebra en el año 2005-, y los artículos publicados en la Revista de Psicología del Deporte en los años comprendidos entre 1995 y 2003. A partir de los datos obtenidos se ofrecen "verdaderas" propuestas de innovación en psicología del deporte, procurando ofrecer "pistas" de lo realmente nuevo en nuestra ciencia.

    Nutritional review of the methodology applied in sports supplementation research

    Both high-performance athletes and amateurs resort to taking supplements in order to improve their performance and/or their health. Many of them do not know the effects of these and take them without a professional supervision. The aim is to know the population who is studied by other different studies related to supplementation and improvement of sports performance and the nutritional method used in these cases. Systematic bibliographic research (March-May2021) was carried out in MEDLINE, with Boolean operators (((((caffeine[MeSH Major Topic]) OR (creatine[MeSH Major Topic])) OR (nitrate[MeSH Major Topic])) OR (sodium bicarbonate[MeSH Major Topic])) AND (dietary supplement[MeSH Terms])) AND (athletic performance[MeSH Terms]). 17 articles were included. Mean participants of these studies were 16.8subjects, which corresponds to a small sample. Only two studies were conducted to women. Several others were based on people who are not considered high-performance. In most of the selected studies, there is no a nutritional assessment, dietary record or follow-up of the subjects’ diet before and during the intervention, and this is a limitation. In order to reach accurate conclusions about supplementation and optimization of sports perfor mance, it is necessary to know the nutritional and hydration status, supplementation habits and lifestyle of the participants in these trials are.

    Panorama temático de la educación médica en América Latina (2020–2025): una revisión de alcance sobre producción científica

    La investigación en la educación médica es un componente esencial para la mejora de la educación de los futuros profesionales de salud, que fortalce el sistema de salud y sanitario del mundo. En esta revisión sistemática exploratoria se examinaron las principales áreas temáticas de publicaciones realizadas por revistas latinoamericanas de investigación en la educación médica indexadas entre los años 2020 a 2025. Para esto, se siguieron las guias PRISMA, en el cual se recopilaron los artículos pertinentes en SciELO, Scopus, Web of Science, LILACS, Redalyc u otras bases relevantes. Tras la selección de acuerdo a nuestros criterios de elegibilidad, se presentaron 1118 artículos y se realizó un análisis temático con ATLAS.ti. Como resultados, los temas predominantes fueron evaluación, competencias, salud mental y rendimiento académico, mientras que áreas como metodologías activas y simulación permanecen subexploradas. Se identificó una tendencia decreciente en la producción científica de educación médica, en contraste con el aumento general de publicaciones en la región, posiblemente relacionado con limitaciones en financiamiento y estructura académica. Además, se evidenciaron diferencias nacionales en prioridades temáticas, con Brasil centrado en salud mental, México en rendimiento académico, y Cuba y Chile en metodologías de enseñanza. Estos hallazgos evidencian la urgencia de ampliar la agenda investigativa regional para fortalecer la educación médica desde una perspectiva más contextual, equitativa e innovadora con el fin de potenciar el proceso de formación de los futuros profesionales de salud.

    Pensions, Ageing and Social Security Research: Literature Review and Global Trends

    Pension systems are one of the fundamental pillars of the welfare state. The ageing of the population caused by longer life expectancy and low birth rates has led to a crisis in the public pension system in developed countries. Changes for the system’s sustainability are necessary, and the scientific literature on the subject is abundant, especially in recent years. This article aims to carry out a bibliometric analysis of the research carried out to date, highlighting, in turn, future lines of research. The study was carried out on a total of 1287 articles published from 1936 to 2021 and found in the Scopus database. The SciMAT, VOSviewer, and Datawrapper tools were used to analyse the most important articles, authors, countries, and institutions by volume of production and citations, as well as the relationships between them. Likewise, the most important keywords and their evolution over time were highlighted, obtaining the main focus of the research. In addition to the general analysis, a specific study was carried out in the area of Mathematics. The results show that the leading countries are the United Kingdom, the USA, and the Netherlands. On the other hand, the lead subject area in which these articles have been published is Economics, Econometrics, and Finance. The research trends are sustainability, pension reform related to ageing, and pension insurance.

    Polymorphous adenocarcinoma of the salivary glands: an overview of immunohistochemical features and insights on molecular pathology

    Polymorphous adenocarcinoma (PAC), represents a common minor salivary gland tumor (SGT) characterized by local growth, low metastatic potential and non-aggressive biologic behavior. Due to the clinical aggressiveness noted in a subset of such tumors, the former term polymorphous low-grade adenocarcinoma (PLGA) was recently revised. PAC’s clinical features and histological diversity result in clinical overlap of this entity with several other SGTs including mainly adenoid cystic carcinoma (AdCC). Differential diagnosis among the entities is crucial, in terms of tumor management and patients’ prognosis. The aim of the present review is to summarize the histological, cytological, immunohistochemical and molecular features of PAC. Histopathological examination is usually adequate for PAC differential diagnosis from other SGTs, except of AdCC. Several immunohistochemical markers including c-Kit, S-100/ MG, Mcm-2 and Integrin β-1, -3, -4, are reported to be useful diagnostic aids in borderline cases. Limitations in sample numbers and study methodology issues of the immunohistochemical PAC studies complicate the identification and selection of appropriate markers useful in the differential diagnosis. Additionally, molecular analyses of PAC specimens indicate that the PAC spectrum phenotypes result from different genotypes (protein kinase D positive; PRKD(+) and PRKD(-) tumors). PAC pathogenesis remains to be determined in each particular genotype while the convergence issue should be addressed in future studies.

Sin embargo, su calidad depende en gran medida de la integridad, preparación y actitud ética de quienes asumen la tarea de evaluar los manuscritos. El presente artículo expone criterios éticos y prácticos para la revisión, destacando la importancia de un proceso riguroso, respetuoso y formativo tanto para autores como para la comunidad científica.

    Social media and television: a bibliographic review based on the Web of Science

    One of the most important trends in current television research has to do with the increasing impact of social media on viewing and consumption. This article reviews the literature on social media and television included in the Web of Science database from 2005 to 2013 in order to find the main research questions guiding academic research within media studies. The topics investigated included the theoretical approach and definition of social television, the central role of Twitter as the main social tool for second-screen activities, the prominence of “reality TV” as the most social genre on television, and new mainstream forms of television fandom. The conclusion of this article reflects on possible limitations of these studies and new paths of research.

    Systematic review of psychosocial benefits obtained with interventions to promote active commuting in schools

    Active commuting to schools has decreased in the last few years. The main objectives of this review were to determine which interventions are being carried out to promote active commuting, to know the quality of the research, and to analyze which psycosocial parameters can be improved. The bibliographic search was carried out in the databases of Medline, Sport Discus, Scopus, Web of Science, and Google Scholar... Finally, there were selecting a total of 23 investigations. The most frequent action is to imple-ment programs to promote active commuting (91.30%). There are few data as yet on interventions, and approximately half of the studies fail to improve active commuting data (47.83%). Psychosocial health is not just studied only 1 study tested variables of psychosocial health. It is important to increase the number of studies, to randomize them, control external contaminants, increase the duration, and increase the psychosocial health studies.

    The ilocaecal junction remains a controversial region of the gut. There are still a lot of questions concerning its anatomical structure and function. In this review, a concise overview is given of the recent and older literature on the embryology, anatomy, including the intrinsic and extrinsic innervation, and the pharmacology of this region. Based on the available data from the literature, evidence is accumulating for a sphincteric function.

    Tumor heterogeneity: morphological, molecular and clinical implications

    Malignant tumors are characterized by their great heterogeneity and variability. There are hundreds of different types of malignant tumors that harbour many oncogenic alterations. The tumor heterogeneity has important morphological, molecular and clinical implications. Except for some hematopoietic and lymphoproliferative processes and small cell infant tumors, there are not specific molecular alterations for most human tumors. In this review we summarize the most important aspects of carcinogenesis and chemoradiosensitivity of malignant cells. In this regard, some oncogenes such as neu , ras and bcl-2 have been associated with cellular resistance to treatment with anticancer agents. The knowledge of oncogenic alterations involved in each tumor can be important to correlate the morphological features, the genetic background, the prognosis and the clinical response to treatment with anticancer agents. Based on the molecular background of the tumor there are new cancer gene therapy protocols. For example using adenovirus Ela in tumors with overexpression of neu oncogene, inhibitors of tirosine kinase specific for the PDGF receptor in glioma, inhibitors of farnesil transferase to prevent ras activity in tumors with mutations in the ras gene.
