    Dental implants in patients with oral lichen planus: a cross-sectional study

    Purpose: The main aim of this study was to evaluate the patients with oral lichen planus (OLP) and dental implants. Material and Methods: Three groups of 16 patients took part in the study. Group I patients had received dental implants and been diagnosed with OLP; Group II had not received implants but were diagnosed with OLP; Group III had implants but not OLP. Clinical observations and OLP symptoms were registered in each case. Periodontal pocket depth, implant mobility, bleeding upon probing, erythema, pain, and radiolucency around implants were measured. Patient quality of life was evaluated using OHIP 14. Results: Peri-implant mucositis and peri-implantitis were detected in 17.86% and 25% of the OLP-implant group, while the control group with implants showed 18% and 16%. The implant survival rate in patients treated for OLP did not appear to differ from the survival rate among the general population. Quality of life was better among patients with implants and without OLP (p = .001). Conclusions: The results of the present study suggest that implants do not influence manifestations of OLP. OLP is not a risk factor for peri-implantitis.

    Effect of Photobiomodulation on Atrophic–Erosive Clinical Forms of Oral Lichen Planus: A Systematic Review

    Abstract: Introduction. Oral lichen planus is a chronic autoimmune inflammatory disease of unknown origin, characterized by various clinical forms of which the atrophic–erosive causes patients the greatest symptomatology. For this reason, there are different treatments that improve the associated signs and symptoms. One of these therapies is photobiomodulation (PBM), which, although new, has a high level of acceptance in dentistry based on evidence. However, there are inconsistent results in its application against lichen planus. The aim of this review was to evaluate the effect of photobiomodulation and its effectiveness as a therapeutic alternative for atrophic–erosive lesions. Material and methods. The databases PubMed, Google Scholar and Cochrane Library were searched to identify studies investigating the photobiomodulation treatment in atrophic–erosive lesions of oral lichen planus. A total of 294 articles were identified, published between 2017 and 2022, and then evaluated; 7 articles that met all the inclusion criteria were included in this study. Results. The type of laser light source used in PBM was the diode laser (four cases), the Nd–YAG laser at the same wavelength of 1064 nm (two cases) and the He–Ne laser (one case). The minimum and maximum wavelengths used were 630 nm and 1064 nm, respectively. Most studies used lesions treated with topical corticosteroids as a control group. The follow-up times of the studies were highly variable. Conclusions. Photobiomodulation is a treatment that competently combats oral lichen planus lesions by improving signs and symptoms, with no known adverse reactions so far, which makes it more beneficial compared to more conventional therapies, such as corticosteroids, for which side effects have been found.

    Eficacia del ácido hialurónico en terapia combinada frente a la monoterapia con corticoides tópicos en el liquen plano oral : impacto en el estrés oxidativo y parámetros salivales

    Objetivos: En este estudio se evaluó la eficacia del ácido hialurónico en terapia combinada con corticoides tópicos en pacientes con liquen plano oral y analizó los marcadores salivales y estrés oxidativo. Métodos: Se diseñó un estudio controlado, aleatorizado y a doble ciego, de acuerdo con los principios de la Declaración de Helsinki y aprobado por la Comisión de Bioética de la Universidad de Murcia. Se han incluido un total de 95 pacientes con edades comprendidas entre 18 y 90 años. De ellos, 35 pacientes sanos (grupo control). Los 60 pacientes restantes, diagnosticados de liquen plano oral (grupo test). Los pacientes del grupo test se han dividido en 3 grupos (20 pacientes por grupo) de forma aleatoria y se les ha administrado el siguiente tratamiento según el grupo: - Grupo 1: Acetónido triamcinolona 0.2% + ácido hialurónico 0.1% en orabase (AT + AH) - Grupo 2: Acetónido triamcinolona 0.2% en orabase (AT) - Grupo 3: Ácido hialurónico 0.1% en orabase (AH) El tratamiento se ha administrado 2 veces al día en la zona afectada durante 1 mes en los tres grupos. Se completaron todos los test realizados, Thongrasom, Arduino, OHIP-14, HAD, recogida de muestra salival, cuestionario de satisfacción y tartrectomía. Resultados: La calidad de vida mejora significativamente independientemente del tratamiento que se utilice. Tanto el AH y AT o monoterapia mejoró todas las variables e índices de calidad de vida Thongransom y EVA respecto a la sintomatología y la calidad de vida OHIP 14. Se tomaron muestras salivales de los pacientes con LPO el primer día antes de hacer la tartrectomía y también después de realizar el tratamiento (1 mes). La tercera muestra salival fue al mes. Los resultados fueron que los marcadores SOD, GSH, CUPRAC, FRAP, TEAC, AOPP, ADA y alfa amilasa tuvieron un cambio significativo a lo largo del ensayo, independientemente del grupo de tratamiento. Se ha observado que los pacientes con liquen plano oral presentan los marcadores salivales CUPRAC, FRAP; TEAC, AOPP, alfa amilasa e IGA podrían tener capacidad discriminativa para diagnosticar el liquen plano oral frente a los pacientes sanos. Conclusiones: Los resultados del presente estudio sugieren que los tres protocolos de aplicación usados mejoran la calidad de vida de los pacientes y fueron efectivos para tratar el LPO sintomático. Además, CUPRAC, FRAP, TEAC, AOPP, alfa amilasa e IGA podrían ser de utilidad en el diagnóstico de LPO.

    Implications on pathogenesis and risk of oral lichen planus neoplastic transformation: an ex-vivo retrospective immunohistochemical study

    Aims. To evaluate OPN, MCM7, Ki-67, p53, Bcl-2 and 53BP1 presence, together with the abnormal adaptive CD4 and CD8 T-cell response markers expression in a series of oral lichen planus (OLP) affected patients and assess their combined contribution for a more objective disease classification. Methods and results. In this ex-vivo retrospective analysis, biopsy specimens from 28 adults with a clinical diagnosis of OLP at different progression degree (16 reticular, 2 plaque-like, 1 erosive and 9 mixed type) were collected. Sections were immunohistochemically investigated for the proinflammatory cytokine osteopontin (OPN), alpha-beta CD4 and CD8 positive T cells, DNA replication licensing factor (MCM7), proliferating cell marker (Ki-67), apoptotic and tumor antigen (p53), apoptosis modulator (Bcl-2) and cellular response regulator to double-strand breaks tumor suppressor p53-binding protein 1 expression. Statistical analysis revealed that 53BP1 is highly represented among the OLP study patients (p<0.05). Moreover, on the basis of the quantification results of the highly expressed parameters, two illness categories with different severity were evidenced. The classification hypothesis was confirmed by i) OLP lesion persistence, ii) the development of oral severe lesions in the patients belonging to high grade activity OLP group (HGA-OLPs) and iii) the ascertainment of the same evidence both in the oral squamous cell tumor controls (OSCC) and in HGA-OLP cases. Conclusion. This study completes the scenario with respect to early detection, thanks to a more precise histological analysis, for rationalizing the clinical and histological findings toward a sharable international disease scoring system.

    miRNA-21 and miRNA-27b expression in saliva of patients with oral lichen planus: a systematic review

    Oral lichen planus (OLP) is a chronic inflammatory disorder of the oral mucosa with a recognized risk of malignant transformation. MicroRNAs, particularly miRNA-21 and miRNA-27b, have been implicated in the pathogenesis and progression of various diseases, including OLP. Their altered expression in saliva may provide diagnostic and prognostic insights for this condition. This systematic review examines the expression profiles of miRNA-21 and miRNA-27b in the saliva of OLP patients to assess their potential as biomarkers. The review was conducted in accordance with PRISMA guidelines and was registered in the PROSPERO database. A comprehensive search was conducted in PubMed, Embase, and Scopus using specific keywords. Retrieved titles and abstracts were screened based on predefined eligibility criteria, and relevant studies were analyzed. The initial search identified 71 studies. After screening, 17 abstracts were selected for full-text review. Following evaluation, 11 studies were excluded, resulting in 6 studies being included. Findings indicate a consistent upregulation of miRNA-21 and a downregulation of miRNA-27b in OLP saliva samples. These alterations suggest a potential role in disease pathogenesis and risk assessment. The dysregulation of miRNA-21 and miRNA-27b in OLP underscores their potential as salivary biomarkers for diagnosis and disease monitoring. Moreover, the non-invasive nature of salivary miRNAs offers promising clinical applications, enhancing early detection and personalized management strategies for OLP.

    Photodynamic Therapy, Photobiomodulation and Acetonide Triamcinolone 0.1% in the Treatment of Oral Lichen Planus: A Randomized Clinical Trial

    Objective: To evaluate the efficacy of photodynamic therapy (PDT) and photobiomodulation (PBM) in the treatment of oral lichen planus (OLP) in comparison with the use of topical corticosteroids. Material and methods: Sixty patients with OLP were randomized to three groups: group 1 photodynamic therapy applied once a week for four sessions, with orabase cream; group 2 low-power laser application with orabase cream; and group 3 inactive laser with triamcinolone acetonide 0.1%. Patient pain was evaluated, and the Thongprasom severity score, the Oral Health Impact Profile-14 (OHIP-14), and the Hamilton anxiety and depression scale at one and three months of follow-up. (ClinicalTrials.gov Identifier: NCT05127083). Results: Pain decreased significantly over time in all groups, though the symptoms relapsed over follow-up at one and three months in group 3. The OHIP-14 score improved significantly in groups 1 and 2 (p < 0.05), and this improvement was maintained after three months. Lesion resolution evaluated by the Thongprasom score at one month showed significant differences between groups 1 and 3 (p = 0.032) and between groups 2 and 3 (p = 0.024). Conclusions: Photodynamic therapy and photobiomodulation once a week for four weeks are safe and non-invasive treatment options, with the important advantage of lacking adverse effects. Further studies are needed to confirm it.

    Premalignant nature of oral lichen planus. A retrospective study of 550 oral lichen planus patients from south-eastern Spain

    The most important complication of oral lichen planus (OLP) is the development of squamous cell carcinoma (SCC) – this being an association that remains subject to controversy. This study aims to examine the incidence and clinical presentation of oral malignancies associated with OLP. A retrospective study was made of 550 patients diagnosed with OLP according to the criteria of the World Health Organization, in the period between 1991 and 2007, in south-eastern Spain 128 males (23.3%) and 422 females (76.7%). A clinical protocol was applied in all cases (sociodemographic data, habits and hepatitis C markers), with histological confirmation of the disease. Five of the 550 patients (0.9%) developed SCC. The mean duration of follow-up was 24 ± 20.83 months. The tongue was the most common location. The exact incidence of malignant transformation of OLP is difficult to establish, due to the possible contribution of external risk factors that may be of relevance in oral malignancy.
