- PublicationOpen AccessAnalysis of the training of 17-18-year-old male powerlifters with cerebral palsy(Universidad de Murcia. The participants of the study were 17-18-year-old male powerlifters with cerebral palsy. The participants were selected and allocated into two groups: Group A and Group B. Both the groups were homogenous in terms of physical and mental state of training. Pre-test and post tests were conducted for physical qualities and mental states of the participants. During the annual experiment, powerlifters in groups А and B performed different training programs with various ratios of specific and non-specific loads. In group A, the ratio was 60% - 40%, while in group B was 70% - 30%. Upon completing the annual experiment, a better (р<0.05) level of physical qualities (speed, speed-strength and static endurance) and mental state of training (neuropsychic resistance, success motivation and motivation to avoid failures) was identified in participants of group A. Therefore, we recommend to use specific and non-specific loads in a ratio of 60% - 40% when training 17-18-year-old male powerlifters with cerebral palsy.
- PublicationOpen AccessCalidad de vida en adultos institucionalizados con parálisis cerebral infantil(Murcia: Servicio de Publicaciones de la Universidad de Murcia, 2020) Martínez-Traver, Laura; Cervera Gasch, ÁguedaIntroduccion: La parálisis cerebral es considerada como un grupo de trastornos permanentes del desarrollo que originan limitaciones en la actividad diaria. Una de las complicaciones en las personas con parálisis cerebral es la disminución de la calidad de vida. Objetivo: El objetivo de este estudio es conocer la calidad de vida de las personas adultas institucionalizadas con parálisis cerebral infantil y conocer si existen diferencias en el índice de calidad de vida en función de las características funcionales. Método: Estudio observacional descriptivo trasversal en personas adultas institucionalizas con parálisis cerebral infantil realizado en los centros APCA y ASPROPACE entre marzo y septiembre de 2017. Se administró el cuestionario San Martín para medir la Calidad de Vida, Gross motor classification system, cuestionario Barthel, Escala CFCS para medir función motora, dependencia física, nivel de comunicación y relacionarlas con variables sociodemográficas y clínicas. La selección de los pariticipantes se llevó a cabo mediante un muestreo estratificado aleatorio. Se contó con la aprobación de las direcciones de los centros. Resultados: Participaron 39 personas con edad media de 32,54 años. La puntuación media global del cuestionario San Martin fue de 102,97 puntos. Las dimensiones mas afectadas fueron bienestar material, desarrollo personal e incluison social. No se obtuvo significación estadística en la puntuación global del cuestionario en función de las variables secundarias. Conclusiones: Los resultados de este estudio han demostrado que la Calidad de vida de adultos con parálisis cerebral en los sujetos estudiados es buena.
- PublicationOpen AccessCaracterísticas físicas y psicomotoras, modalidades de tratamiento y escolarización en distintos tipos de parálisis cerebral infantil en la Región de Murcia(Universidad de Murcia, 1994) García-Sánchez, Francisco Alberto; Caballero, Presentación A.; Castellanos, PilarEn este estudio se han analizado las características físicas y psicomotoras de 48 niños con parálisis cerebral infantil de tipo hemipléjico, dipléjico, tetrapléjico e hipotónico, las modalidades de tratamiento recibidas en el Centro de Atención Temprana de ASTRAPACE (Murcia) y el tipo de escolarización alcanzado. Los resultados obtenidos apoyan conclusiones de estudios epidemiológicos previos y señalan la existencia de importantes diferencias en el perfil psicomotor de los distintos síndromes de parálisis cerebral.

Estrategia de búsqueda Revisión sistemática en bases de datos PEDro, PubMed, Enfispo, Dialnet, IME (índice médico español), Science Direct e Isi Web Of Knowledge mediante términos Mesh y el operador lógico AND, así como una búsqueda bibliográfica manual. Selección de estudios y datos Fueron seleccionados 5 artículos que cumplían los criterios de inclusión y exclusión establecidos. Resultados Todos los estudios cuantificaron una mejora tras la intervención, ya fuese en variables fisiológicas, en el comportamiento o en la movilidad funcional; sin embargo, la calidad metodológica era escasa, siendo dos de ellos estudios de un solo caso, otros dos presentaban un tamaño muestral superior pero sin grupo control y un quinto estudio carecía de muestra aleatoria. Conclusiones Son necesarios estudios de mayor calidad metodológica que permitan evidenciar los efectos reales producidos por la hidroterapia en este ámbito.
- PublicationOpen AccessEffect of lumbar-pelvic control on functionality in children with spastic cerebral palsy(Universidad de Murcia, Servicio de Publicaciones, 2023) Ahmed Hussein, Zeinab; Mohammed Mousa, Nehad; Soliman Ali, MostafaLack of pelvic control is a common functional impairment in children with spastic cerebral palsy (CP), so pelvic stability may be critical to improvecontrolled movements and activities in these children.The aim of this randomized controlled trialwas to evaluate the effect of physical therapy programs combined with pelvic control exercises on controlled hip, knee, and ankle movements and gross motor functions in children with spastic CP. Thirty-two children diagnosed with spastic cerebral palsy at levels II and III on the gross motor functional classification system participated in this study. Theywere randomly and equally divided by a computerized program into control and study groups. The control group (eight boys and eight girls) received the designed therapy program for one hour. The study group (four boys and twelve girls) received lumbar-pelvic control exercises for 20 minutes in addition to 40 minutes of a physical therapy program. The program for both groups was applied three times per week for twelve weeks. After twelve weeks of treatment, the controlled movement, functional ability plus a joint range of motion of the hip, knee, and ankle were assessed by selective motor control scale, Peabody scales, and Kinovia software program, respectively.All statistical measures were performed through the Statistical Package for Social Studies (SPSS) version 20 for windows (SPSS, Inc., Chicago, IL). The results of our studyshowed a statistically significant improvement in median selective motor control, the Z score of Peabody scale, and the mean angle of hip, knee, and ankle range of motion after the treatment program in the study group compared with the control group (p < 0.05).Lumbar-pelvic control exercises are useful in improving the controlled movement and functional abilities in children with spastic diplegiaCP when combined with a physical therapy program.
- PublicationOpen AccessEffect of treadmill training combined with ankle weight on balance in spastic cerebral palsied children: A randomized controlled trial(2024) Wagdy, Eman; Adel Ahmed, Angham M.; Mohammed Hassan, Manal; Ahmed Kamal, Ola; Aziz, Marwa A.; Mouhamed, Hanady A.The aim of this study was to examine the impact of treadmill training combined with ankle weights on balance and dorsiflexor muscle strength in children with spastic hemiparetic cerebral palsy (CP). This clinical, randomized trial included forty boys and girls suffering from spastic hemiparetic cerebral palsy, ranging in age from 6 to 9 years old. They were split into two equal groups (control group = 20, study group = 20). Both groups received a program of physiotherapy in addition to treadmill exercise, while the study group also received ankle weights for eight weeks. The Biodex stability system was employed to evaluate stability indices, including the overall stability index (OASI), anteroposterior stability index (APSI), and mediolateral stability index (MLSI). Functional balance was measured using the Pediatric Balance Scale (PBS). A Lafayette manual muscle tester (MMT) was used to record the peak force of ankle dorsiflexor muscle strength for both groups, pre- and post-therapy. Post-therapy results showed a significant decrease in all stability indices at stability levels 8 and 4, and a significant increase was observed in functional balance and ankle dorsiflexor muscle strength in both groups (p<0.05). The study group showed a more significant improvement in all outcome measures after therapy than the control group (p<0.05). The program of physiotherapy, treadmill training, and ankle weights was effective in improving balance and dorsiflexor muscle strength among children with spastic hemiparetic cerebral palsy, with ankle weights contributing to greater progress.
- PublicationOpen AccessEfficacy of massage and stretching for controlling hamstring muscle spasticity in children with spastic diplegia cerebral palsy: A comparative study(Universidad de Murcia. Objective: This study aimed to explore the impact of stretching alone or in combination with short-term massage on reducing hamstring muscle spasticity and enhancing knee range of motion (ROM) in children with spastic diplegia cerebral palsy. Methods: Twelve children diagnosed with spastic diplegia cerebral palsy (6–12 years old) were randomly divided into two groups: the first received massage and stretching exercises, and the second received stretching exercises only on the hamstring muscle. The Modified Ashworth Scale was used to measure the stiffness of the hamstring muscle, and the ROM of knee flexion was measured using a manual goniometer. ROM and modified Ashworth scale (MAS) were measured at baseline and after 1 and 2 weeks of treatment, respectively. Both legs of each participant received treatment for 2 weeks, with the duration of massage being 5 min and the duration of stretching being 30s with three repetitions. Results: Both methods reduced hamstring spasticity after two weeks; however, ROM improved after the first week and more significantly after the 2nd week. Conclusion: Stretching exercises, whether performed alone or in combination with short-term massage, have been found to be equally effective for treating spastic muscles. Although there was an improvement in spasticity after the first week of short-term massage, the difference was not statistically significant.
- PublicationOpen AccessIntervención terapéutica y coordinación familiar en los inicios comunicativos de un caso de atención temprana(Universidad de Murcia. Servicio de Publicaciones, 2020) Vidal Esteve, María Isabel; Peirats Chacón, JoséEn este estudio de caso se aborda la intervención realizada en un centro de diagnóstico y atención temprana, a una alumna de dos años con hemiparesia lateral izquierda y dificultades en la manipulación y comunicación. Aplicando a lo largo de cuatro meses el sistema de Comunicación Total de Benson Schaeffer con el apoyo del método TEACCH, se pretende superar las graves limitaciones comunicativas de la alumna. El procedimiento metodológico utilizado es mixto y las técnicas de investigación empleadas son la aplicación de un test estandarizado, la entrevista, los registros de observación y el análisis documental. A partir del test cumplimentado por la madre que establece la evaluación inicial del caso, se diseña y aplica el programa realizando registros sistemáticos, al final de la intervención se vuelve a cumplimentar y se comparan los datos conseguidos. Además, se entrevista a la cuidadora de la niña y se analiza la documentación aportada por la familia y el centro donde es atendida. Los resultados obtenidos muestran que se han logrado grandes beneficios en cuanto a aprendizaje de los signos, comunicación de necesidades, emisión de verbalizaciones y generalización en los distintos contextos, fomentando en la niña una mayor autonomía y estableciendo unas bases sólidas sobre las que poder sustentar nuevos aprendizajes.
- PublicationRestrictedIs the implementation of Vojta therapy associated with faster gross motor development in children with cerebral palsy?(LifeTime Media Ltd, 2021-09-30) Sanz Mengíbar, José Manuel; Menéndez Pardiñas, Mónica; Santonja Medina, Fernando; Cirugía, Pediatría y Obstetricia y GinecologíaBackground and purpose – Vojta therapy has been reported as clinically beneficial for strength, movement and gross motor activities in individual cases and is being included within the second of three levels of evidence in interventions for cerebral palsy. The goal of this study is to understand the effect of Vojta therapy on the gross motor function. Methods – Our clinical trial followed a one group, prepost design to quantify rates of changes in GMFM-88 after a two-months period undergoing Vojta therapy. Results – A total of 16 patients were recruited. Post-intervention acceleration rates of GMFM-88-items acquisition (0.005; p<0.001) and Locomotor Stages (1.063; p<0.0001) increased significatively following Vojta therapy intervention. Conclusion – In this study, Vojta therapy has shown to accelerate the acquisition of GMFM-88-items and Locomotor Stages in children with cerebral palsy younger than 18 months. Because functional training was not utilised, and other non-Vojta therapy intervention did not influence the outcome, Vojta therapy seems to activate the postural control required to achieve uncompleted GMFM-88-items. ------------------
- PublicationOpen AccessModulation of the angle of rigid ankle-foot orthosis to control knee hyperextension in children with unilateral cerebral palsy(Universidad de Murcia, Servicio de Publicaciones, 2023) Shafeek, Marian M.; Aly, Sobhy M.; Ahmed, Yomna F.; Ibrahim, Hoda M.The aim of this study was to compare the effects of a modified 5° dorsiflexion ankle-foot orthosis (AFO) to the traditional right angle rigid AFO on controlling knee hyperextension and improving spatiotemporal gait parameters in children with unilateral cerebral palsy (CP). We used a pretest-posttest experimental design in which forty children (2-6 years) of both genders with unilateral CP were randomly assigned into two equal groups (A and B). Group A used a traditional right angle rigid AFO and received a selective exercise program to enhance walking pattern while group B used a modified 5° dorsiflexion rigid AFO and received the same exercise program as group A. Assessments for the knee angle during mid-stance and spatiotemporal gait parameters were done pre and post-intervention programs for both groups. All statistical analyses were conducted using the Statistical Package for Social Studies (SPSS) version 25 for windows (IBM SPSS, Chicago, IL, USA). There was a significant decrease in the knee angle in mid-stance post-treatment in groups A and B compared to pre-treatment (p < 0.001) but a significant increase in spatiotemporal gait parameters post-treatment in groups A and B compared to pre-treatment (p < 0.001). There was a significant decrease in the knee angle in mid-stance of group B compared to group A in post-treatment (p > 0.001). However, there was no significant difference in spatiotemporal gait parameters between groups post treatment (p > 0.05). In conclusion, using a rigid AFO improves the spatiotemporal gait parameters and decreases knee hyperextension for children with unilateral CP. Furthermore, a modified 5° dorsiflexion rigid AFO is recommended for better control of knee hyperextension than a traditional right angle rigid AFO for such cases.

This study aimed to quantify improvements in participation, as well as complexity on task-related manual activities in children with severe bilateral CP. This analytic quasi-experimental study exposed 17 children with severe cerebral palsy to 13 NMT sessions to improve motor learning through therapeutic instrumental music performance (TIMP), using principally percussion musical instruments. Hoisan software video recording was used to quantify participation involved in creating music. In addition, the number of active movements performed in each NMT session was quantified. Significant improvements were found in the participation variables “visual contact,” “motor participation” and “motor participation repetitions.” Significant differences were also found in the subcategory “reaching and stroke,” “hitting with the hand” and “grasping and hitting.” The use of therapeutic of TIMP in children with severe CP improves participation during manual activities utilizing percussion instruments, therefore increasing the intensity of the psychomotor intervention.
- PublicationOpen AccessPercepción de carga de un grupo de cuidadores de adolescentes con parálisis cerebral(Universidad de Murcia. Servicio de publicaciones, 2022) Bolaños Roldan, Ana MarcelaIntroducción: La parálisis cerebral es una condición neurológica irreversible. Se describe como un grupo de trastornos de postura y movimiento causados por una lesión en el cerebro inmaduro; no es progresiva y puede estar asociada a problemas perceptuales, sensoriales, de comunicación o de conducta. La experiencia de cuidado crea cambios significativos tanto en la vida familiar como en las estructuras sociales en las que conviven. Objetivo: El objetivo de este estudio fue conocer la percepción de carga en cuidadores de adolescentes con parálisis cerebral severamente comprometidos, espásticos en su mayor proporción, a partir de aspectos sociodemográficos y clínicos. Materiales y métodos: Estudio cuantitativo transversal. Se aplicaron dos cuestionarios con información sociodemográfica y clínica en la que se usó la Clasificación de la función motora gruesa a 60 adolescentes con parálisis cerebral y la Escala de carga de Zarit a sus cuidadores. Resultados: El 55% de los pacientes fue de sexo masculino. La edad media fue 16 años, el 70% eran cuadriparéticos y el 75% espásticos. El 48% de los cuidadores percibieron sobrecarga intensa. También se encontró que el estrato socioeconómico tiene una relación negativa estadísticamente significativa con el nivel de carga percibido (p=0,0262). El 45% de los cuidadores perciben que debido al tiempo que pasan con su familiar, no tienen suficiente tiempo para sí mismos. Conclusiones: Los cuidadores perciben alta dependencia por parte de los pacientes e insuficiencia de recursos económicos para el cuidado; adicionalmente refieren que su situación familiar o relaciones interpersonales no se ven afectados a causa del cuidado.
- PublicationOpen AccessResearch on the Cognitive Orientation to daily Occupational Performance Approach: a bibliometric review(American Occupational Therapy Association, 2024-10-25) Valera Gran, Desirée; Delgado Lobete, Laura; Montes Montes, Rebeca; Navarrete Muñoz, Eva María; Atención Sociosanitaria; Facultades de la UMU::Facultad de Ciencias Sociosanitarias (Lorca)Importance: The Cognitive Orientation to daily Occupational Performance (CO-OP) is an evidence-based intervention approach that has significantly increased in popularity over the past two decades. However, how the research literature on this topic is patterned is still unknown, so it is difficult to identify potential areas for research and clinical interest. Aims: To analyze the literature published on the CO-OP approach to provide a detailed and structured analysis of the publication patterns. Data Sources: All literature related to CO-OP included in the Web of Science database through June 15, 2024. Study Selection and Data Collection: All published studies related to CO-OP were included in the bibliometric analysis, which was conducted on the raw data retrieved from the Web of Science database using the Bibliometrix R package. Findings: The annual growth rate in CO-OP research has been 0.8%, significantly increasing from 2015 onward. Most of the research has been published in occupational therapy and rehabilitation journals by English-speaking research teams, and its scope has expanded from developmental coordination disorder to a wide range of health conditions. In addition, two main lines of research have emerged: one focusing on understanding the underlying cognitive processes involved in CO-OP and the other focusing on its clinical effectiveness. Conclusions and Relevance: Research on CO-OP has significantly increased over the past decade, and it currently encompasses a wide range of areas. This analysis may facilitate the advancement of research on and the clinical practical application of CO-OP. Plain-Language Summary: The Cognitive Orientation to daily Occupational Performance (CO-OP) is an evidence-based intervention approach that has significantly increased in popularity over the past two decades. Occupational therapists use it to improve the occupational performance of both children and adults with movement difficulties. However, it is unknown how the research literature on this topic is patterned, so it is difficult to identify potential areas for research and clinical interest. This study found that scientific literature on CO-OP has significantly increased in the past decade and that its scope has expanded from developmental coordination disorder to a wider range of health conditions, such as cerebral palsy. Moreover, research is now focused on two main questions: What are the underlying cognitive processes involved during intervention, and how effective is the CO-OP approach? These findings can be used to further improve occupational performance and participation among occupational therapy clients who struggle with motor performance and planning.
