- PublicationOpen AccessAplicabilidas del rediseño de procesos en el ámbito hospitalario público.(Murcia : Servicio de Publicaciones de la Universidad de Murcia, 2007) Giménez Andreu, María del Rosario; Segura López, GabrielLas nuevas tendencias gerenciales en la administración publica sanitaria abogan por un mayor protagonismo de los ciudadanos en la toma de decisiones de los procesos asistenciales y por una mejora de la calidad de los servicios hospitalarios ofrecidos. Conscientes de esta realidad, las direcciones hospitalarias se están caracterizando por el creciente interés en desarrollar proyectos de mejora de sus procesos asistenciales básicos, utilizando para ello en algunos casos herramientas como la denominada Reingeniería o Rediseño de Procesos que supone una demolición controlada y drástica de los procesos estratégicos de valor agregado y de los sistemas, las políticas y las estructuras organizacionales que los sustentan para optimizar los flujos del trabajo y la actividad de una organización. En este artículo estudiamos el Rediseño de Procesos desde un punto conceptual, revisando las fases necesarias para su puesta en marcha y analizando su aplicabilidad potencial en el ámbito Hospitalario Público. Concluimos resaltando que la práctica del rediseño de procesos no es, según sus principios conceptuales, factible desde un punto de vista ejecutivo en los hospitales públicos, lo que nos indica que las prácticas así denominadas, rigurosamente no pueden ser tales en realidad y se inclinan más bien a otras orientaciones organizativas.

Design/methodology/approach Private hospital organizations have been selected using a judgmental sampling. A qualitative case study was followed in this research. Findings The findings shed light on corporate planning of future sustainability initiatives in private healthcare organizations. Research limitations/implications The diversity of similarities and differences that has been uncovered between private healthcare organizations on corporate planning, reveals the complexity faced in trying to achieve sector-wide and or industry-wide uniformity of sustainability initiatives. Managerial implications These findings offer opportunities to examine criteria to examine the corporate planning of future efforts and priorities in private healthcare sectors across countries and continents. Originality/value This paper distinguishes between corporate planning approaches in relation to the assessment criteria to examine future sustainability initiatives in private hospitals.

- PublicationOpen AccessLos cuidados enfermeros de San Juan de Dios según el Modelo de las necesidades humanas de Henderson(Universidad de Alicante, 2023-12-11) Muñoz Devesa, Aarón; Morales Moreno, Isabel; Bermejo Higuera, José Carlos; Galán González-Serna, José María; Atención SociosanitariaConsulting manuals of the History of Nursing, it can be appreciated how it is from Nightingale when considering the birth of modern or professional Nursing. However, this belief could be a black legend, because before the 19th century there was already awareness of Nursing as a profession, being this initiated by Juan de Dios, the one from Granada, in the Spanish 16th century. For this reason, in this article we have set the objective of describing the care of Juan de Dios following the model of human needs described by Henderson, together with care in agony and post-mortem. To do this, we use the historiographical methodology following the current of the history of mentalities, so that we can go from the past to the present, and vice versa. In the documentary analysis we have been able to observe how what we find today systematized in a theoretical way was already carried out at the beginning of the nursing reform of the Spanish 16th century, being a valid model for current Nursing, since it is found in its being, knowing , and doing, the concepts of the current nursing metaparadigm, although not explicitly, since its intention, more than theoretical, was care, although behind it there was a corpus of doctrine independent of other socio-health disciplines. -------------------------

- PublicationOpen AccessDeterminación de los factores de estrés de los enfermeros que actúan en una unidad de internación.(Murcia : Servicio de Publicaciones de la Universidad de Murcia, 2005) Menzani, G.; Ferraz Bianchi, E.R.La enfermería es conocida mundialmente como una profesión altamente estresante, pero no presenta factores de estrés idénticos en todas sus áreas de actuación. El presente estudio tiene como objetivo investigar los estresores presentes en la actuación de los enfermeros en unidades de internación. Fue realizado con una población de enfermeros de una institución pública y gubernamental, siendo utilizado un cuestionario para la recogida de datos, con el fin de conocer los factores de estrés de esos enfermeros y compararlos con otras unidades de actuación. Como resultados, identificamos que los factores de estrés más citados son los relacionados con las condiciones de trabajo de los enfermeros y de la administración de personal. No existió una correlación estadísticamente significativa entre las áreas de estudio del estrés y la actuación del enfermero y el tipo de unidad (unidad de interación en relación a las unidades de cuidado intensivo y semi-intensivo). Hay la necesidad de otros estudios para la comprobación de los resultados presentados.
- PublicationOpen AccessHerramientas telemáticas en aulas hospitalarias: una experiencia educativa en la Región de Murcia(2013-07-29) Serrano Sánchez, José Luis; Prendes Espinosa, Mª Paz; Negre Bennàsar, Francisca; Didáctica y Organización EscolarEsta investigación tiene como principal objetivo mejorar la atención educativa del alumnado hospitalizado gracias al uso de las Tecnologías de la Información y la Comunicación (TIC). Para ello se ha diseñado una propuesta de integración curricular de TIC, que tras su implementación y evaluación puede ser adoptada por el profesorado de las aulas hospitalarias. En dicha propuesta tiene especial relevancia la creación de la herramienta telemática ALTER, que permite gestionar recursos educativos digitales en red, herramientas Web 2.0, y actividades. Supone un aporte importante en las aparentes deficiencias metodológicas que existen en la comunidad científica de la Pedagogía Hospitalaria en relación a los procesos de integración educativa de las TIC. El contexto de estudio ha sido las aulas hospitalarias de la Región de Murcia (España). Estos espacios educativos flexibles tienen como principal finalidad ofrecer apoyo educativo a los niños hospitalizados, además de ayudar a prevenir el posible aislamiento provocado por la hospitalización.
- PublicationOpen AccessHistoria del Santo y Real Hospital de Caridad de Cartagena (1900-1936).(2013-01-23) Sánchez Martínez, José; Marset Campos, Pedro; Guillén Pérez, José Jesús; Departamentos y Servicios::Departamentos de la UMU::Ciencias SociosanitariasEl Hospital de Caridad de Cartagena fue fundado en 1693. Objetivo: determinar su área de influencia, los problemas sanitarios, enfermedades y lesiones más frecuentes y otros aspectos. La fuente principal ha sido el archivo del Hospital. Entre 1900 y 1936 el Hospital experimenta un gran auge. El 58% de enfermos proceden de la Ciudad, siguiéndole la zona minera (19%). Existe una correlación negativa entre los ingresos y la estancia media. Enfermedades más frecuentes: aparato digestivo (21%), respiratorias (15%), circulatorias (10%) y genitourinarias (9%). Enfermedades infecto-contagiosas más frecuentes: tuberculosis (20%), difteria (19%), paludismo (17%), sífilis (16%), fiebres tifoideas (4%) y viruela (3%). La tasa más elevada de hospitalización por paludismo corresponde a la zona húmeda de “El Hondón”. Existe una correlación negativa entre consumo de pan y de carne por estancia y número de estancias. Entre las urgencias predominan las heridas (59%), traumatismos osteoarticulares (24,3%), quemaduras (3,3%) y cuerpos extraños (2,6%). The “Hospital de Caridad” in Cartagena was founded in 1693. Objective: to determine its area of influence, health problems, most common diseases and injuries and other aspects. The archive of the Hospital has been the main information source. Between 1900 and 1936 the Hospital experiences a great development. 58% of patients are from the City followed by those from the mining zone (19%). A negative correlation was observed between admissions and average length of inpatient hospital stay. Most frequent diseases: digestive system (21%), respiratory system (15%), cardiovascular system (10%) and genitourinary system (9%). Most frequent infect-contagious diseases: tuberculosis (20%), diphtheria (19%), malaria (17%), syphilis (16%), typhoid fever (4%) and smallpox (3%). The main zone affected by malaria is the wetland area called “El Hondón”. A negative correlation was observed between bread and meat consumption per hospital stay and average length of stay. Most common emergencies are wounds (59%), osteoarticular traumatisms (24.3%), burns (3.3) and foreign bodies (2.6%).
- PublicationOpen AccessImpact of the COVID-19 pandemic on the hospital: Inpatient’s perceived quality in Spain(SAGE Publications, 2021-03-03) Vidal-Abarca, Inmaculada; Beteta, Dolores; López-Ibáñez, Mercedes; García-Vázquez, Elisa; López-Picazo Ferrer, Julio José; MedicinaAlthough the Coronavirus disease 2019 (COVID-19) pandemic has generated a large amount of studies, the patient-perceived quality of care (PQ) in this context is still not well known, so more studies intending to focus on this issue are strongly needed. This study assesses changes on PQ in patients hospitalized in Spain during the first month of the COVID-19 pandemic and investigates differences between those admitted for this cause and the rest a descriptive study using the “Net Promoters Score” and the hospital regular monitoring plan. Due to this point of view, ethical approval is not applicable. Four PQ dimensions (nurse, physician, and nurse assistant actions [NA], and discharge information [DI]) were measured in all COVID patients (57) and in a sample of non-COVID patients (60) discharged at home during the first month of the pandemic, and also compared with another sample (384) from an immediately previous period. The COVID patients scored worse (8.2) than non-COVID ones (9.0; P < .0001), especially in NA and DI, and were more likely to be detractors (odds ratio [OR]: 3.05, P < .0001) and less to be promoters (OR: 0.64, P < .05). Global and DI net promoters score values before the pandemic were higher than afterward. In conclusion, the COVID-19 pandemic negatively and significantly influenced the health care quality as perceived by inpatients, both in COVID and in non-COVID ones, but more intensely in the former. As a health care organization, this knowledge meant an opportunity from improvement and to be better qualified to face the pandemic.
- PublicationOpen AccessOnline tools in the hospital classrooms of the Región of Murcia(2012) Serrano Sánchez, José Luis; Didáctica y Organización EscolarThe investigation presented has a conclusion of improving the educative attention that children who find themselves in the care of hospital receive during considerable amount of time (and that during this period, they attend classes overseen by the hospital) through the use of ICT. After undergoing the investigations and experiments related to the topic, we come to the conclusion that in the majority of cases, the playful use of ICT in the hospital classrooms dominates the educative. Through this investigation, our intention is to form a module of curricular integration with ICT, through the creation of an online tool that categorizes diverse digital educational resources and tools with the use of Web 2.0, that can have educative applications in the hospital classroom. This educative experiment originates from the collaboration between investigators from different universities, experts in educative technology and the teaching staff from hospital classrooms in the region of Murcia. In this assignment we present the actions taken, the phase in which right now we find the project and the actions yet to be completed.

Objective: The objective of this study was to relate nursing workloads to professional job satisfaction. Methods: This is an observational, analytical, descriptive, concurrent and quantitative study, which used the Overall Job Satisfaction scale and subscales therein, to identify global satisfaction—intrinsic or related to motivational factors and extrinsic or associated with hygienic factors—in nursing professionals (n = 104) from eight Inpatient Units of Internal Medicine and Surgery, in four hospital centers, to describe job satisfaction in the professionals studied and to find statistically significant associations between job satisfaction and workload (measured with the scale MIDENF®) in the inpatient units where they work. Results: There were higher levels of satisfaction in the variables “relationship with immediate boss” and “relationship with fellow workers”, and lower levels in “relationship with senior management” and “organizational system of the unit”. In the inferential analysis, the scores were 75.63 for overall satisfaction, 35.28 for intrinsic satisfaction, and 40.36 for extrinsic satisfaction. Conclusions: There is a close relationship between workload and job satisfaction, showing more dissatisfaction regarding organizational aspects and professional recognition.
- PublicationOpen AccessThe aerobiome in a hospital environment: characterization, seasonal tendencies and the effect of window opening ventilation(Elsevier, 2023-01-16) Núñez, Andrés; García, Ana M.; Genética y MicrobiologíaThe urban atmosphere carries biological particles (bioaerosols) that may cause several diseases and allergies. These bioaerosols infiltrate and mix with those present inside the buildings, including hospitals. However, little is known about the behavior of these particles around health facilities. Here, we described the bioaerosols composition of an urban hospital indoor and outdoor at two different periods (winter and summer) using DNA sequencing. We observed that the seasonality and composition of the bioaerosols outdoor was also displayed indoor, and, in some cases, the taxa showed different trends depending on the season. Pathogenic species of bacteria and fungi were found indoors at low levels but also outdoors, being mostly environmental species, which would reject the idea that hospitals may be acting as a source of emission via aerosols. Skin-related bacteria were the most prevalent group related to human microbiome, being more abundant indoors. Air temperature was the principal factor affecting the bioaerosols composition in the samples but, in general, meteorological parameters outdoors were poor descriptors of the bioaerosols indoors. Similarly, the concentrations of the main indoor air pollutants did not correlate with microbial abundances. Globally, natural ventilation through a window opening did not significantly alter the composition of the bioaerosols indoor.

J.; EnfermeríaAbstract: We conducted validation of a scale to measure nursing workloads, previously designed using NIC interventions within the four nursing functions (patient care, teaching, management, and research). Methods: This is an analytical, descriptive, prospective, and observational study using qualitative methodology (focus groups and in-depth interviews) with a quantitative and qualitative section (committee of experts and real application of the scale through a validation pilot and with multicentric application, including hospitalization units of internal medicine and surgery of four hospitals). Qualitative analysis was performed with Atlas.ti8 and quantitative analysis with R. Results: Qualitatively, all the participants agreed on the need to measure workloads in all nursing functions with standardized terminology. The expert committee found greater relevance (91.67%) in “prevention” and “health education” as well as consistency with the construct and adequate wording in 99% of the selected items. In the pilot test and multicenter application, the nurses spent more time on the caring dimension, in the morning shift, and on the items “self-care”, “medication”, “health education”, “care of invasive procedures”, “wounds care”, “comfort”, and “fluid therapy”. Cronbach’s alpha 0.727, composite reliability 0.685, AVE 0.099, and omega coefficient 0.704 were all acceptable. Construct validity: KMO 0.5 and Bartlett’s test were significant. Conclusions: The scale can be considered valid to measure nursing workloads, both qualitatively in obtaining the consensus of experts and health personnel and quantitatively, with acceptable reliability and validity superior to other similar scales.
- PublicationOpen AccessValoración de la necesidad de extensión de los servicios de Atención Temprana al ámbito hospitalario(Universidad de Murcia, 1998) Caballero García, Presentación A.; García-Sánchez, Francisco A.En este trabajo analizamos la información obtenida por 147 encuestas administradas a padres de niños con necesidades de Atención Temprana. Dichas encuestas hacían referencia a las hospitalizaciones requeridas por el niño en sus primeros 6 años de vida. Recogían información sobre hospitalización neonatal y hospitalizaciones esporádicas o reiteradas postnatales, su duración, motivos de ingreso y posibles repercusiones a nivel conductual y psicosomático. Los resultados indican que estos niños requieren con frecuencia hospitalizaciones de larga duración, tanto neonatales como por encima de los dos años de edad. Se concluye la necesidad de arbitrar mecanismos eficaces para la coordinación y cooperación entre los servicios de intervención temprana que atienden al niño y los servicios de pediatría y enfermería hospitalarios, con el fin de dar cierta continuidad a la atención que recibe el niño y paliar posibles consecuencias negativas de la hospitalización en estas edades claves de la infancia.
